Who You Are
We are seeking a dedicated professional to join our Hospital Discharge Team at Highgate East Hospital. You will work closely in a multi-disciplinary mental health team, collaborating with NHS colleagues and other agencies to support service users with mental health disorders and dementia. Your role will involve managing caseloads, assessing and reviewing needs, and safeguarding vulnerable individuals.
