What the Job Involves
In this role, you will support the delivery of health and social care functions for adults and carers in Wirral, focusing on personalisation and social inclusion. You'll work within a multi-disciplinary team, supporting the duty function, and participating in safeguarding processes. Responsibilities include conducting needs-led assessments, coordinating support plans, and ensuring compliance with professional standards. You will also provide guidance and supervision to newly qualified social workers and engage in community forums and initiatives.
