You are a compassionate individual with experience working with homeless or vulnerable adults who have support needs. You have a strong background in risk assessment, support planning, and key working. Collaborating effectively with safeguarding, health, social care, and housing services comes naturally to you. You are dedicated to supporting residents in achieving positive outcomes and moving towards independent living.

What the job involves

The Supported Pathways Officer provides person-centred support to vulnerable adults with a history of homelessness or rough sleeping. The role focuses on assisting residents to transition into long-term stable accommodation while preventing homelessness. Responsibilities include providing client-centred support to maintain current accommodation, facilitating service connections and activities for wellbeing, conducting interviews and risk assessments for new residents, delivering intensive support for independent living, and contributing to the daily delivery of hostel services. The role operates Monday to Friday on a shift pattern, without weekend or bank holiday work.
