Who You Are
A temporary Locum Social Worker is sought to contribute to community work through planned reviews and reassessments with residents and their families. You should be familiar with relevant legislation and have experience working with older and vulnerable adults in community and hospital settings.
What the job involves
The role involves coordinating and completing integrated assessments, hospital discharge planning, and maintaining high-quality assessments and care planning. You will also support the public-facing 'Information, Advice & Assistance Service' and work within a multi-disciplinary team.
Skills
- Knowledge of Social Services & Well-being (Wales) Act 2014
- Understanding of Mental Capacity Act 2005
- Adult Safeguarding procedures
- Experience with older and vulnerable adults
- IT and communication skills
- Experience with ‘Flo’ database (desirable)
- Multi-disciplinary team experience
