Who You Are
You are an experienced social worker with a solid background in multidisciplinary social care and strong learning disability practice. You demonstrate empathy, sensitivity, and resilience while maintaining professionalism and embracing organizational change. You possess a recognized Social Work qualification and are registered with Social Work England.
What the Job Involves
This role involves undertaking detailed Care Act reviews and reassessments for adults with learning disabilities, participating in face-to-face visits, and gathering evidence to assess current care and support needs. You will work independently across Somerset, attend the office a minimum of two days per week, and collaborate with various stakeholders. You are responsible for producing high-quality and audit-ready assessments and ensuring that practice remains person-centred and compliant with statutory responsibilities.
