Social Worker (HAY BD B)

Who You Are

We are seeking an experienced and compassionate social worker familiar with the Discharge to Assess processes and hospital discharge pathways. You should have working knowledge of the Care Act 2014 and the Mental Capacity Act 2005. You are someone who can practice in a strength-based way, promoting independence, and reducing reliance on formal services. You should have excellent assessment skills, be risk-aware, and be adept at making defensible decisions. Additionally, you should be a team player with the ability to work independently, manage pressure, and meet deadlines.

What the job involves

This role involves working within the Urgent and Emergency Care South Team, focusing on patient discharge in hospital settings across South Nottinghamshire. Responsibilities include applying care legislation to practice, collaborating with multidisciplinary teams, and ensuring timely hospital discharges. You will participate in Decision Support Tool meetings and support individuals in accessing reablement services and community support, all while maintaining high standards of care and decision-making.
