We are seeking a highly motivated and qualified social worker to join the Lambeth Hospital Social Work Team at St Thomas’ Hospital. You should be adaptable to a demanding and changing environment, committed to partnership and collaborative working, and passionate about achieving positive outcomes for our residents. Experience with adults over 18 years with physical health disabilities and vulnerabilities is essential, as well as a robust understanding of safeguarding procedures.
What the Job Involves
As a Hospital Social Worker, you will be responsible for delivering strength-based preventative support and safeguarding patients. You will assess and establish support systems, referring individuals to reablement where appropriate, and ensuring safe and timely facilitation of discharges. You will undertake reviews of existing care packages, considering placements, and collaborate closely with multidisciplinary teams and voluntary sector organizations. The role supports hybrid working, requiring on-site presence at St Thomas’ Hospital for 3 days per week, with an option to work 2 days from home.

Note: The rate for this position is £32 LTD per hour.
