What the Job Involves
As part of the Pathway for Adults (18-25yrs) team, popularly known as the transition team, you will support young adults transitioning from children's services to adult services. You will collaborate with health colleagues like speech and language therapists, occupational therapists, learning disability nurses, psychologists, behavior therapists, and psychiatrists. Your responsibilities will include completing Care Act Assessments, Mental Capacity Assessments (MCA), Best Interest Decisions (BID), and more importantly, ensuring our service users, diagnosed with Learning Disabilities (LD), Autism, ADHD, Global Developmental Delay (GDD), and other physical/sensory impairments, receive comprehensive support.
