Who You Are
You are a registered Social Worker with a passion for working with older people, utilizing your post-qualifying experience to assess and manage the complex needs of adults and their carers. You are committed to promoting health, wellbeing, and social inclusion, and are adept at working collaboratively with Reablement and other professionals. You are proactive in safeguarding and promoting independence and have a holistic approach to assessing needs.
What the Job Involves
In this role, you will be responsible for assessing and managing the needs of adults and their carers, ensuring their health and social well-being. You'll maintain a small but complex caseload, investigate abuse, and promote equality and diversity. The role requires you to collaborate effectively with other agencies, conduct Mental Capacity Act assessments, and ensure accurate and timely case recording. You will also prepare court and specialist reports, mentor newly qualified practitioners, and adhere to health and safety guidelines.
