We are looking for two enthusiastic and qualified Social Workers to join our East and South Locality Teams within the Adults and Communities Directorate at Slough Borough Council. You have a strong commitment to working with adults and their families, providing excellent evidence-based practice, particularly for older people and adults with physical disabilities. You possess the drive and enthusiasm to deliver our vision of thriving, diverse communities.
What the Job Involves
As a Social Worker in a multi-disciplinary team, you will apply a strength-based, community-focused model of social care. You will hold a complex caseload of residents or their carers, conducting assessments, care and support plans, and reviews as per legislation and council procedures. Your role will involve facilitating service user self-assessments, risk assessments, and coordinating with other professionals. You will also contribute to joint working practices within Community Locality teams and be responsible for recording data, adhering to financial procedures, safeguarding work, and supporting clients to maintain independence.

- Strong interpersonal communication skills
- Proficiency in risk and mental capacity assessments
- Ability to work with diverse communities
- Efficient case management and workload prioritization
- Knowledge of the Care Act and safeguarding practices
- Effective partnership and relationship building
- Experience with Telecare and assessment tools
- Financial competency in support planning
- Current HCPC registration and relevant degree
- Full UK Driving Licence and access to a car
