Who You Are

We are looking for a qualified social worker with experience in mental health to join our team as a Placement Review and Step-Down Specialist. The ideal candidate will have prior experience in health or social care management, with specific knowledge relating to housing and supported living facilities for individuals with severe mental health conditions. The candidate should be capable of working independently to set objectives, as well as cooperatively in a multidisciplinary setting, aiming to support clients in achieving better outcomes and maximizing their independence.

What the job involves

This role involves reviewing supported living placements primarily in coordination with health partners. The focus is on facilitating step-down placements to enhance clients' independence and ensuring optimal outcomes. The position requires assessing accommodation needs for clients with severe mental health issues, providing effective case management, building and maintaining collaborative relations across various organizational and professional boundaries, and ensuring compliance with relevant mental health policies.
