You are a dedicated and qualified social worker with a passion for supporting older people who are vulnerable or need support. You are committed to promoting independence and ensuring that citizens, their families, and carers are at the center of decision-making about their lives and outcomes.
What the Job Involves
Your role involves delivering a high-quality social work service to older people. You will work alongside citizens, carers, and families to develop resilience and independence, ensuring compliance with statutory timescales for assessment and review. Your responsibilities include establishing partnerships with various stakeholders, safeguarding citizens, conducting risk assessments, and maintaining professional accountability in resource allocation. You will also be involved in the community wellbeing initiatives and support citizens in participating in service development.
Skills
- Recognised Social Work qualification, 3 years post-qualified
- Registered with Social Care Wales
- Knowledge of current legislation and guidance
- Person-Centred Planning approaches
- Experience with complex case work and safeguarding procedures
- Ability to communicate effectively in English and Welsh
- IT literate, proficient in Microsoft packages
- Effective teamwork and ability to manage a varied caseload
- Commitment to innovative and evidence-based practices
