As an Outreach Family Support Worker, you are someone who is passionate about improving the lives of children and families. You thrive in dynamic environments and are adept at coordinating support with multi-agency partners. You approach your work using a strengths-based and whole-family strategy, aiming to address emerging concerns and facilitate positive change.

What the Job Involves

Your role entails providing high-quality outreach and home-based support to children and families. You will take case responsibility, acting as the Lead Professional to deliver targeted support and prevent the need for statutory services. Responsibilities include conducting family help assessments, creating support plans, and chairing team meetings to review progress. You will work directly with children and young people to identify their views and concerns, ensuring that interventions improve their outcomes. The role also involves maintaining effective records and aligning work with the Signs of Safety model.
