You will take case responsibility, acting as the Lead Professional to deliver targeted support and prevent the need for statutory services. Responsibilities include conducting family help assessments, creating support plans, and chairing team meetings to review progress. You will work directly with children and young people to identify their views and concerns, ensuring that interventions improve their outcomes. The role also involves maintaining effective records and aligning work with the Signs of Safety model.
