You have demonstrated abilities in working with adults and carers, particularly in improving their well-being and participation. You excel in partnership with other health and social care agencies and have a profound understanding of utilizing community resources effectively.
What the Job Involves
This role involves listening and connecting with adults and carers to promote well-being and independence. You will carry out and review assessments, support planning, and promote opportunities for the professional and practice development of peers. Collaborating with multi-disciplinary teams, engaging other professionals, and ensuring a multi-agency approach to independence is key. Statutory requirements under the Care Act 2014 and Mental Health legislation are to be met, along with safeguarding responsibilities and Court representation when necessary.
